The Highland Person Centred Planning Group, which is supported by ARC Scotland, hosted a conference on the 7th November 2008 in Inverness.
The Conference, It's My Life, gave several organisations from the Highlands and from other areas in Scotland the opportunity to showcase good practice in transitions.
Transitions
As part of the It's My Life Conference, Marea Foulis, Transitions Coordinator from the Highland Council told the conference about three documents:
- Its My Journey
- Transitions Guide booklet for children and young people
- Transitions Guide booklet for parents, professionals and partner services
Highland Children's Forum were the lead in the consultation It's MY Journey which then informed the Transitions Guides and the Integrated Services for Young People and Young Adults with Additional Support Needs as a result of Disability and/or Complex Health Joint Transitions Policy and Joint Transitions Procedure.

'It's MY Journey' informed and advised work undertaken in collaboration with a range of professionals from partner agencies in Highland to produce the Transistions Guides:
Pilot Study
The documents were piloted in schools and with partner services between February and May 08. They were subsequently amended as a result of the valuable feedback regarding their usefulness.

The Transition Guides offer children and young people the opportunity to plan for transition regardless of age or stage and provide transition information, ideas and resources to their supporters for enabling children and young people to be active partners in their own transition plans.
Getting it Right for Every Child
The Guides are intended to enhance processes now live in children's services in Highland, through the implementation of GIRFEC (Getting it Right for Every Child), for ensuring and demonstrating that we are getting the right support to the right children at the right time and engaging and enabling them to fully participate in their own transition planning.
